Blended Format Booking Form

Use this form to book your Diploma in Cognitive Behavioural Hypnotherapy if
You want to book a MIX of Online and Webcast format for each Stage
(e.g. Stage 1 Webcast, Stage 2 Online, Stage 3 Webcast)

INSTRUCTIONS

1/ For each Stage of the Diploma simply choose the format you wish to take it in (Webcast or Online)
2/ Then choose your dates from the drop down
3/ Then choose your payment schedule


There is a £400 saving if you select pay in full.
There is a further £100 Early Bird saving if you are booking more than 30 days before the start date of Stage 1 and you are paying in full.
You can select your payment plan schedule for 12 months.
Early Bird rates do not apply to Blended Bookings that start with an Online Diploma.
Please note payment plans longer than 12 months may require a full credit check and references.

Note: when selecting “ONLINE” for Stage 2 or Stage 3 there are no set dates. You will be given access to Stage 2 or Stage 3 when you have completed the previous Stage (either by Webcast or Online).

Note: please do not select a Webcast date for Stage 2 that is within 30 days of an Online Stage 1 intake date. You will not have enough time to be ready to join the Stage 2 Webcast group and you will not be able to join. Allow 45-60 days to complete an Online Stage.
